This season, expect more emotion, tears and joy as life long dreams and fairy tale fantasies come true. Some of the incredible makeovers you will see include two sisters, who have struggled all their lives with cleft palettes and undergone nearly 40 surgeries, now turning to their last hope to be normal — the Extreme Makeover team; a colorful bull rider who's had his teeth knocked out and wants to be transformed into an urban cowboy; a goth punk rocker woman who has spent her life hiding behind her appearance getting the program's first "make-under," as the extreme team tones down her shocking style; and in another first, the show will make over an entire family (children will be styled with hair, makeup and wardrobe without surgery). New procedures never before seen on the show will be performed, such as a reverse vasectomy for a man who wishes to have another child with his wife and a hair transplant for a balding young woman. Plus hear the perspectives of these medical challenges as told by the doctors, and see more updates on some of last season's most popular makeover participants.

In addition, for the first time, participants will recuperate at the "Makeover Mansion," the new luxurious mansion tucked away in the Hollywood Hills, complete with stunning views, a swimming pool, fully equipped home gym and plasma televisions. Also, continuing with the mission to help people realize their dreams, "Extreme Makeover" will grant the wishes of several participants: an aspiring actress who is flown to Manhattan to attend New York Fashion Week, complete with a dress designed especially for her by the famed Nicole Miller; a girl, long ridiculed for her looks, unveiled for the first time with her makeover at her high school reunion; and a talented violinist who lands the gig of a lifetime — to play with her idol, singing superstar Leann Rimes.

Each new episode features participants in their "before" phase, then as they undergo their various procedures and, finally, when the end results are revealed to their families and friends. The transformations are accomplished through the skills of an "Extreme Team," consisting of plastic surgeons, dermatologists, eye surgeons and cosmetic dentists, along with a talented team of hair and makeup artists, stylists and personal trainers. In addition, resident style guru Sam Saboura performs a "Mini-Extreme Makeover," a fast and furious wardrobe/hair/makeup overhaul for a lucky individual, with results revealed in the first half-hour of the show. Sam dispenses his tricks and tips in the fields of fashion, personal style and grooming with "news you can use" to give yourself a whole new look without any plastic surgery.
