Not much, unless you pierce it slowly with a safety pin like I did my second earhole. >< Otherwise, not really. My friend pierced it for me with a little... snappy thing. It only hurt for a second, but not the scary kind of pain. Also, afterwards, I kept being afraid that my ear would rip because the earring felt so lose and wierd, but after a while I got used to it. You just have to make sure to clean it every day.
